15 Year Old Traveling From Canada To the USA via Greyhound?

If I would be travelling from Toronto to New York, NY for a short while and am 15 years old, what documents would I need? I know that my parents would have to sign a form which states that I could travel myself (does it have to be notarized?), but what other documents would I need?

1. Kids 15 and under may travel with just their birth certificate. You also need a letter of permission. See site below.
2. You need a valid passport and yes, the letter from your parents (both of them) has to be notarized.
